New Patient Registration
The practice accepts patients from the following postcode areas:
- PA6
- PA10 (Kilbarchan village only)
- PA11
- PA13
From Friday 1 September 2017, the practice will no longer accept new patients from the PA5 postcode area. This change does not affect patients who live in Brookfield who were registered before this date.
When registering you will be asked to complete a form for each person registering with the practice.
In addition you will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, ie while your medical records are transferred from your old practice to this one, and a questionnaire indicating ethnicity and language needs.
Forms are available below to download and all three should be given to reception staff when registering. Along with your forms, please bring with you for each person a birth certificate or passport to confirm your date of birth and, for adults, a driving licence or utility bill to confirm your address. We will not retain copies of these.
If you move out of the practice area, you will need to register with a practice in your new area as soon as you move.
New patients will be requested to make an appointment to see the practice nurse or, if a patient is on a complicated medication regime or takes a lot of medication, a GP appointment will be made.
