Access to Medical Records
You are entitled to information contained in your medical record, but we ask that you put the request in writing to the practice manager so that (a) it is clear exactly what you are requesting, and (b) the practice has an accurate record of what has been given to you.
The practice is entitled to charge for the information you request. The current charge is £1.00 per page*. This is to cover staff and any GP time as well as paper and printing costs. If the information you request is held on computer, the maximum we will charge you is £10.00. We ask that you collect the information requested at the surgery of your choice and pay the fee at the time of collection.
The practice has forty days to provide this information to patients, but we will normally provide it well within this timescale. We will contact you when the information is available for collection.
Please note that, if you request information and do not collect it, we will send you an invoice for the fee.

*This charge does not apply to requests for copies of your immunisation record or private letters which you may ask your GP for, as there is work involved in researching and/or dictating/typing this information. Our secretaries or reception staff can give you details about fees.
